Q: What type of incident can I report?
A: The Incident Response form is only valid for the following 

  • Acts of Discrimination
  • Discriminatory Harassment
    • Sexual Harassment
    • Physical Harassment
    • Verbal Harassment
    • Psychological Harassment

Q: Who will be on the recipient side of this form?
A: The MUS Ethics Board and Ombudsperson. Your submission will remain highly confidential.

This form can be filled out by anyone for incidents that occured in the MUS context, including MUS events. For incidents outside the scope of the MUS, please refer to other faculty’s incident response teams.
The Ombudsperson can be contacted to assist you through these processes should you choose.
